Instructions for Registration for the Active 2006 conference

To register for the Active 2006 conference please download the registration form:
- MS Word format: Edit the document using MS Word and complete all the fields. Use the 'Tab' key to change between the fields.
- PDF format: Print the form and complete all fields using legible handwriting in BLOCK LETTERS.

Print the form. The form is formatted for A4 paper size. If you are printing to 'Letter' paper size change the scale of the document as follows:
- If you are using MS Word, under File | Print, in the Zoom section in the bottom right corner, change 'Scale to Paper Size' to 'Letter'.
- If you are using Adobe Acrobat, under File | Print, in the 'Paper Handling' section, change 'Page Scaling' to 'Fit to Printer Margins', and also select 'Auto-Rotate and Center'.
- Get the credit card holder to sign the form to authorise the payment of the conference fees.
Note, if you would prefer to pay by direct bank transfer, please send an email to the registration address listed below to ask for the banking details. Unfortunately we are no longer able to process registration fees by bank transfers and will only accept payment by credit card.

- Delegates should organise their own accommodation arrangements. Special arrangements have been made with a number of hotels that are within 10 minute walking distance of the conference venue. Please see the Accommodation pages on this web site for the list of suggested hotels, prices, and a map showing the location of each hotel relative to the conference venue.

Refunds - less $A50 administration fee - will be granted to delegates unable to attend, provided WRITTEN notice is received by AAS by 23 June 2006.
No refunds will be granted for cancellations received after 23 June 2006.
Reimbursements will be made after the congress.
